- Geospatial Solutions for Digestive Health in Rural CommunitiesGeographic barriers in rural areas lead residents to postpone or forgo healthcare entirely, especially for specialized care. One solution to address this accessibility issue is telehealth. At Intermountain Health, rural patients use telehealth for specialties like gastrointestinal (GI) and digestive health. To improve preventive care, we used GIS to help predict where patients are most likely to develop GI and digestive health issues. We created a predictor index allowing us to identify areas to expand telehealth services, improve patient education, and equip clinicians for specialized care.

- The Role of Geography and Social Drivers of Health in Readmission RiskHospital readmissions, a patient’s home address, and social drivers of health (SDoH) contribute to poor health outcomes and increased cost. A readmission risk tool incorporating demographics and clinical characteristics to direct case management interventions demonstrated decreased readmission rates and costs. However, there is limited data on the effect of including geography and SDoH factors in the risk tool during hospitalization. We compared the effects of institution-specific readmission rates by neighborhood and composite measures of area-level deprivation on the risk of readmission.
